1. Birth Certificate of the child (PSA Copy or Certified True Copy)
   

2. Marriage Contract of Parents (if married)
   

3. Endorsement Letter from the Parish where you belong (if not residing in barangays 637, 638, 639, & 640)
   

4. Accomplished Baptismal Data and Reservation Form
     
 

Additional Requirement for Two Years Old or above:
   

5. Certificate of No Baptismal  Record from 3 (Three) Churches near your place

9am - Pre-Baptism Seminar (Parents & God Parents)
   

10am - Holy Mass
   

11am - Baptism Proper
