2. Marriage Contract of Parents (if married)
   

3. Endorsement Letter from the Parish where you belong (if not residing in barangays 637, 638, 639, & 640)
   

4. Accomplished Baptismal Data and Reservation Form
     
 

Additional Requirement for Two Years Old or above:
   

5. Certificate of No Baptismal  Record from 3 (Three) Churches near your place

10am - Holy Mass
